Your best approach is to find a qualified designer who will be able to listen to your needs and translate that into drawings for permits and work with a contractor to make sure its buildable and affordable.
Before you call anyone though you need to carefully spend time deciding what it is that you NEED from the extension. Pay attention to the inside and out of the existing house. Identify what views you want to keep, where your daylight will come from, how you will enter the room and what the spaces will be used for and how the traffic will flow from one to the other.
Then spend some time ripping out of magazines images of how you would like the space to look, that will speak volumes to a designer and contractor in understanding the quality of finishes your are looking for and what you are after.
The third item is to review how much money you have to spend, you will be asked by both the designer and the contractor and it isn't in your best interest to be vague or unsure. A designer or contractor can build a 15x20 room that can cost little or a lot, we need to understand where your funding is to make sure we can work with you to design and build something you will be happy with and can afford. If your desires are outside of your budget range discussions need to take place as to whether it is best to wait and save a little more or to execute the project in phases, or to par down the scope of the work to make it more affordable.
